Product Acknowledgement

I acknowledge I have been prescribed the medicinal cannabis flower to be used by the approved oral route (tea consumption).
I have been advised that this product does not meet (has not been assessed for) the minimum quality standards for use by vaporisation. The abovementioned quality standards differ in between routes of administration, chiefly in the acceptance criteria for microbiological quality.
I acknowledge that if I proceed to use the medicinal cannabis flower via the unapproved route of vaporisation, that I do so of my own accord and have been given an opportunity to discuss this with my doctor and ask any questions.
I acknowledge that I have been advised, as below, that the risks of using the medicinal cannabis flower via vaporisation include inter alia: 1. uncertainty whether excipients in an oral formulation are safe to be vaporised and inhaled; 2. uncertain risks regarding the potential that by-products may be produced during the heating process; and 3. the difference in microbial contamination testing limits between oral and inhaled products.
I accept that if I do proceed to use the medicinal cannabis flower by vaporisation I do so at my own risk.
