Patient Declaration and Consent
- The patient will provide background medical notes before the consultation in accordance with NZ Medical Council requirements and understands that not doing so may result in our clinician seeking further information, prior to issuing a script.
- The patient will provide a copy of their photo ID. Patient ID can be an under-18 card, passport, or driver’s license.
- A consultation with one of our doctors does not guarantee a prescription for medicinal cannabis.
- Medicinal cannabis is a section 29 unapproved medicine in New Zealand which means that it has not been approved by Medsafe for a particular condition. This means it is prescribed on a case-by-case trial basis, there is no guarantee there will be any benefit, and there is a possibility of unknown risks and long-term side effects.
- The prescribing doctor’s recommended dosage must be followed and any side effects should be reported as soon as possible (such as lethargy, fatigue, dry mouth, nausea, vomiting, diarrhea, dizziness, disorientation, agitation, balance problems, changes in memory, paranoid delusions, or hallucinations.)
- The patient will be required to attend follow-up consultations with the prescribing doctor from time to time as required by the Medical Council.
- There can be no driving, alcohol, or operating heavy machinery (for at least 8-10 hours) after undergoing treatment with medicinal cannabis containing THC.
- The patient may be prescribed medicinal cannabis products that do not meet the Ministry of Health quality standards and the patient accepts to do so at their own risk.
- The patient acknowledge that information regarding the supply of medicinal cannabis will be forwarded onto the Ministry of Health in accordance with the Medicines Act 1981.
- The patient agree to not use any other forms of cannabis other than what is prescribed by their doctor (this includes any illicit forms of cannabis).
- The patient is aware that conventional and first line treatment options exist for their symptom(s) and should speak to their GP about these options.
- The patient will notify their GP of any changes in other medications
- Patients have the right to choose where they wish to have their prescription filled. The patient may take their prescription to any pharmacy of their choice or opt to use our clinic’s online dispensary service. If the patient chooses to order through our online dispensary, the patient agrees to an independent pharmacy dispensing the prescription. This option is provided for the patient’s convenience and ensures that their medication is handled with the utmost care and professionalism.
